Usually when you register, a campaign is created for you using your username as a code, however if a campaign of that name was already created by someone else then you won't be able to use your username as the code anymore, and you will need to create a new campaign with a new name.

Open the affiliate campaigns page and you will see a create button, simply click on it and enter the campaign name you wish to create. Note that campaign name is unique and it can not be created more than once, so if the name you select is also used, then select another name.
